You are now at Step 2 - Intake. At this
Step, all parties complete
the ElectronicCourthouse Intake
Form. The Intake Form provides basic information about you, the other party, and your
dispute. This information allows us to open a case file and assist the
parties to select the
Resolution Professional that's right for your dispute. Once I have
received the Intake Form of the both parties, I will open a case file, then
send an email message with instructions to both parties on how to proceed to the next
step.
In order to fully protect your privacy, the Intake Form is password protected. Therefore,
in order to complete the Intake Form, please provide us with the information below. I will
then email a user name and password to you to provide you with secure access to your
confidential Intake Form.
To receive your user name
and password to access the secure Intake Form, enter your company name,
your name, contact number and email address.
*** Please note that the
consent of both parties is required to participate in the The Electronic
Courthouse ADR process. When both parties have consented to use
The ElectronicCourthouse ADR process, usernames and passwords will be
sent to the email address you supply below.
